The Quality Engineer was frustrated.

Every Monday morning, the first batch from one assembly station had the same defect.

By Tuesday, the problem disappeared.

The team had already retrained the operator.

They had displayed warning notices.

They had even introduced an additional inspection.

Nothing changed.

The Production Manager decided to observe the Monday shift.

He noticed something interesting.

On Fridays, the operator left a special tool setting ready for the next production run.

But over the weekend, the maintenance team reset the machine for another product trial.

On Monday, the operator assumed the original setting was still in place.

Nobody had informed him about the change.

The problem wasn’t a lack of skill.

It was a gap in communication and process control.

The team introduced a simple setting checklist and a verification step before the first production batch.

The Monday defects disappeared.

This is an example of Poka-Yoke (Mistake-Proofing).

The idea is to design checks and controls that prevent common mistakes or detect them before they create defects.

Training people is important. But even experienced people can forget, assume or misunderstand.

A reliable process should protect against predictable human mistakes.
